What can you expect?
We have an exciting opportunity for a Renal Dialysis Coordinator with a passion for supporting patient care to join our team at our Norwich base, with regular travel to Norfolk hospitals. In this role, you will ensure renal patients have caring, safe, timely and efficient transport prior to and after discharge. You will coordinate seamlessly with hospital and transport teams to improve each patient's journey experience.
The main aim of this role is to utilise the resources available in the best possible way to ensure our renal patients who are based in Norfolk, have the best possible experience using our transport.

* £25,116 per annum
* 37.5 hours per week - Monday to Friday
* Sick pay of 70% salary for up to 6-weeks in a rolling 12-month period for staff who have a full year of service.

Key Responsibilities:
* Streamline Patient Flow: Work closely with control and planning teams to coordinate renal patient discharges and outpatient schedules, minimizing delays and enhancing patient experience.
* Direct Point of Contact: Maintain clear communication with renal units and hospital teams, updating patient readiness, transport timing, and minimizing potential issues.
* Maximize Transport Efficiency: Prioritize transport needs, optimizing resources to support the contract with meeting Key Performance Indicators (KPIs) and contractual standards.
* Confidential Data Management: Update patient information across systems accurately, ensuring confidentiality in line with HTG-UK's policies.
* Patient and Family Support: Act as a liaison for patients' families and care facilities, ensuring clear communication on transport and post-discharge care.
Who You'll Work With:
Engage daily with Site Duty Managers, Renal Unit Managers, HTG Crews, and our Planning Team, as well as patient families and care providers.
Ideal Candidate:
There is a requirement to drive, so having a full UK manual driving licence is essential for the role.
This role is ideal for someone with experience in patient transport or healthcare logistics, who has a commitment to patient-centred care, and strong problem-solving skills. You will have a good work ethic, with excellent communication skills and attention to detail. This role will require the successful candidate to have good IT skills.
