We are looking for a Patient Pathway Coordinator who can help us deliver an efficient, high-quality medical appointments service across our Long Term and Unscheduled Care Directorate.
The Successful Candidate Will
* Provide efficient and effective administration services for outpatient bookings and patient pathway management.
* Prioritise workload to ensure registration of all referrals and booking of appointments align with the Trust's waiting time standards and Patient Access Policy.
* Work according to defined procedures to maximise clinic capacity and help meet waiting time targets.
* Accurately record information into systems and maintain patient appointment data.
This role offers valuable insight into the NHS and requires excellent interpersonal skills for communication with patients and staff at all levels within the Trust.
Team Location and Collaboration
The Patient Pathway Team is based in the Long Term and Unscheduled Care Directorate. They work closely with management to deliver effective outpatient services and support patients.
Team Responsibilities
* Managing appointments and referrals, supporting clinical triage, and booking outpatient appointments.
* Understanding clinic templates and monitoring utilisation.
* Monitoring waiting lists and performing validation.
* Communicating with patients via letter and telephone.
* Building relationships with clinicians, nurses, managers, and admin teams.
