We are looking to recruit an enthusiastic and motivatedpractitioner with initiative and drive. Our 0-19 service delivers high qualitypublic health nursing, that promotes and improves health inequalities to children,young people and their families in Southend-On-Sea. You will need to haveexcellent communication and organisational skills, be willing to facilitate andsupport change in service delivery, have the ability to work autonomously andas part of an integrated skill mix team.
As a health visitor working for Southend City Council you will have access to iHV corporate membership, you will be able to claim NMC professional fees and benefit from the Local Authority pay award each year.
Main duties of the job
You will be required to lead the delivery of the Healthy Child Programme and promote the health of all children and young people, especially those from disadvantaged groups, to ensure they are offered the opportunity to achieve optimum development.
To Identify, prioritise, develop and implement programmes of care to meet individual and local needs. This will be achieved through evidence based assessments, effective care planning and implementation of a range of support packages for individual families, targeting the most vulnerable families and groups utilising best available evidence.
Work in accordance to the commissioned service specification, to ensure the 5 mandated reviews are completed within required timescales, services provided are evidence based practice.

To clinically interpret information, analyse and identifyappropriate individual plans of care in line with Universal, Targeted andor Specilaist cases within the Health Visiting caseload.
To appropriately delegate and supervise workloadwithin the skill mix team in line with service operating procedures and competencyframeworks.
To provide expert advice, support and interventions to allfamilies with children in the first years of life.
To identify the needs of individual children, parents andfamilies (including safeguarding needs) and refer or direct them to existinglocal services, thereby promoting early intervention.
To operate within Children & Young People Services ChildSafeguarding Policies, Standards and Procedures, recognising and takingappropriate action in relation to children where there is suspicion of abuse.
To participate in strategy meetings, case conferences,providing reports as required and to have clinical and safeguarding supervisionas per policy for Safeguarding children.
Liaise and maintain good working relationships withstatutory and voluntary agencies whilst making referrals and consulting asnecessary.
Be competent as a non-medical prescriber.

Essential
1. Effective oral / written communication and interpersonal skills.
2. Skills in managing own workload through effective use of prioritisation.
3. Ability to use evidence based, clinically effective interventions in the holistic planning and implementation of family centred care.
4. Ability to travel extensively within acceptable time scales.
5. Ability to work alone and as part of a team.
Desirable
6. Experience as a Registered Health Visitor.
7. Experience of developing and managing groups.
8. Computer literacy, knowledge of systmOne.
