This is an exciting opportunity for an Advanced Nurse Practitioner to support patients with urgent on-the-day care needs in the local community. This fixed term role for 6 months (secondment considered) to cover maternity leave will support the General Practitioners and the Multi-disciplinary Team to deliver safe, effective and person-centred care to patients in a primary care setting. Successful applicants may work in GP practices, visit residents in care homes or attend to patients in their own homes. There is also a commitment to the Four Pillars of Advanced Practice, allowing for the development of skills in education, mentoring, audit, leadership, and quality improvement.

Applicants will therefore be required to be working in an Advanced Practice role and have undertaken MSC (level 11) Advance Practice studies to PG Dip level or have equivalent experience in an advanced practice role.

Applicants will require excellent communication skills and be able to work as a member of the wider Health and Social Care Team. They should have the ability to work autonomously, utilising their own initiative, clinical judgement and decision making to support the nursing workforce within the practices.
