Candidate Responsibilities – Social Worker (Adults) Conduct comprehensive assessments of adults' social care needs. Develop tailored care plans in collaboration with service users and their families. Provide advice and advocacy to ensure individuals access appropriate services. Liaise with healthcare professionals, families, and external agencies. Maintain accurate and timely case records in compliance with policies. Support and empower adults to achieve greater independence. Attend multi-disciplinary team meetings and contribute to care reviews. Ensure all safeguarding procedures are followed diligently.
