What you will do:
As a Social Worker, your responsibilities will include:
– Undertaking a range of community care and safeguarding assessments to identify risks and needs in accordance with statutory requirements.
– Developing and implementing care plans tailored to client needs, pinpointing any unmet requirements.
– Constructing value for money care packages and commissioning services that minimise risk.
– Working closely with individuals, their families, carers, and communities to support informed decision-making.
– Managing an allocated caseload, ensuring effective monitoring and review of client needs.
– Fostering relationships with clients, offering advice, promoting independence, and providing conflict resolution.
– Liaising with colleagues and external agencies for informed assessment and care planning, advocating for clients and carers.
– Maintaining accurate records and report writing, with court representation as required.
– Supervising social work students, trainees, or support staff, guiding them through practical experiences.
– Continuing post-qualifying professional development in line with the Care Council for Wales requirements.
