Responsibilities
* Conduct comprehensive social work assessments to identify patients' needs and develop personalised discharge plans.
* Collaborate effectively with multidisciplinary teams, including medical staff, nurses, therapists, and community services.
* Arrange appropriate care packages, including home care, equipment, and financial support.
* Provide information, advice, and advocacy to patients and their families.
* Safeguard vulnerable adults and children, adhering to relevant legislation and policies.
* Maintain accurate records and contribute to service development.
* Work to prevent delayed discharges and promote patient independence.
Qualifications and registrations
* Qualified Social Worker.
* SSSC registered as a Social Worker.
Skills
* Car driver licence.
* At least 3 years hospital discharge experience.
