Health Village & Dearne Primary Care Network (PCN) are looking torecruit an Nurse Practitioner with a Minor Illness qualification tocomplement our current workforce within the PCN.

Health Village & Dearne PCN are a partnership of 3 practices, StAnn's Medical Centre, Market Surgery and Clifton Medical Centre. We have apatient population of 48,000 and have a diverse team of staff working withinthe PCN and the practices.


Main duties of the job

* Contribute to chronic disease management by assessing, planning, monitoring and reviewing progress.
* Identify, assess and diagnose patients with complex health needs across the Practice population
* Prescribe products to meet the needs of patients within the Practice population
* Support development of programmes to improve health and well-being and meet the specific needs of communities, groups and individuals
* Work with others to change policies to improve health and well-being and reduce inequalities
* Support the implementation and evaluation of evidence-based practice to improve the services offered by the Practice nursing team
* Support the team in quality assurance, development and clinical governance.


About us

Health Village and Dearne PCN is a collaboration of 3 practices basedacross Rotherham. We are an effective and efficient PCN with a successful trackrecord of providing safe, effective and holistic patient care.

You will work with a varied team of practice staff and other PCN staff.


Job responsibilities

JobPurpose:

The purpose of this post is thedelivery of high quality care to the practice population. You will work withpractice policies, protocols and guidelines whilst using experience andclinical judgement to provide nursing interventions and care.

Working within the NMC code thepost holder will deliver care that is evidence based and cost effective.

To be responsive to patientsneeds and contribute to improving quality standards.

Work collaboratively andautonomously with the multi-disciplinary practice team to meet the needs of thepatients

ClinicalResponsibilities:

Contribute to chronic disease management by assessing, planning, monitoring and reviewing progress.

Identify, assess and diagnosepatients with complex health needs across the Practice population

Prescribe products to meet theneeds of patients within the Practice population

Support development ofprogrammes to improve health and well-being and meet the specific needs ofcommunities, groups and individuals

Support the team in qualityassurance, development and clinical governance.

Maintain all associated records,both clinical and manual.

Communication:

Communicate effectively withpatients/families/carers recognising the need for alternative methods ofcommunication to overcome levels of understanding, cultural background andpreferred ways of communication.

Demonstrate excellentinterpersonal, verbal and written communication skills.

Work collaboratively with themulti-disciplinary team and other external organisations.

Demonstrate sensitivecommunication styles to ensure patients are fully informed and consent totreatment

Communicate with and supportpatients receiving bad news

Anticipate barriers tocommunication and take action to improve communication.

Act as an advocate whenrepresenting patients and colleagues.

Personal/Professional

Recognise and work within owncompetence and professional code of conduct as regulated by the NMC

Produce accurate,contemporaneous and complete records of patient consultation, consistent withlegislation, policies and procedures

Prioritise, organise and manageown workload in a manner that maintains and promotes quality

Deliver care according to NICEguidelines and evidence-based care

Assess effectiveness of caredelivery through self and peer review, benchmarking and formal evaluation

Participate in the maintenanceof quality governance systems and processes across the practice

Utilise the audit cycle as ameans of evaluating the quality of the work of self and the team, implementingimprovements where required

Collaborate on improving thequality of health care in partnership with other clinical teams, responding tolocal and national policies and initiatives as appropriate

Evaluate the patients responseto health care provision and the effectiveness of care

Support and participate inshared learning across the practice and wider organisation

Participate in the managementand review of patient complaints, and identify learning from clinical incidentsand near-miss events using a structured framework (e g root-cause analysis)

Assess the impact of policyimplementation on care delivery

Participate in the performancemonitoring review of the team, providing feedback as appropriate

Understand and apply legalpolicy that supports the identification of vulnerable and abused adults, beingaware of statutory vulnerable adult health procedure and local guidance.

Leadershippersonal and people development

Take responsibility for owndevelopment learning and performance including participating in clinicalsupervision and acting as a positive role model

Support the development ofothers in order to maximise potential

Actively promote the workplaceas a learning environment, encouraging everyone to learn from each other andfrom external good practice

Encourage others to makerealistic self-assessment of their application of knowledge and skills,challenging any complacency and actions that are not in the interest of thepublic and/or users of services

Ensure safe storage, rotationand disposal of vaccines and drugs. Oversee the monitoring, stock control anddocumentation of controlled drug usage according to legal requirements

Support members of the nursingteam to undertake mandatory and statutory training requirements

Apply infection-control measureswithin the practice according to local and national guidelines

Apply policies that reduceenvironmental health risks, are culturally sensitive and increase access tohealth care for all

Participate in the localimplementation strategies that are aligned to the values and culture of generalpractice.

Use technology as an aid tomanagement in planning, implementation and monitoring, presenting andcommunicating information

Review and process data usingaccurate Read codes to ensure easy and accurate retrieval for monitoring andaudit processes

Manage information searchesusing the internet and local library databases, for example, the retrieval ofrelevant information for patients on their condition

Understand the responsibility ofself and others regarding the Freedom of Information Act

Collate, analyse and presentclinical data and information to the team using appropriate charts and/orgraphs to enhance care

Learningand Development

Act as a mentor for more juniorstaff and students, assessing competence against set standards as requested

Disseminate learning andinformation gained to other team members in order to share good practice andinform others about current and future developments

Assess own learning needs andundertake learning as appropriate

Provide an educational role topatients, carers, families and colleagues in an environment that facilitateslearning
