SUPPORT PLANNER THE HOMELESS FAMILIES SERVICE, NOTTINGHAM, PERMANENT, 37HRS, GRADE 3, £25,235,FTE An exciting opportunity has arisen for a Support Planner to work within The Homeless Families Service to support Homeless Families with low support needs. The service provides temporary supported accommodation for 32 families, 9 familiesare housed within the hub, staff are also based here. The remaining 26 families are living in dispersed properties within the Nottingham City boundary. Referrals to the service come through the Local Authority, we work closely with the Local Authority to reduce the use of BB accommodation for often vulnerable families. We are looking for workers who respond well to an often-fast paced service and like a challenge. Support will focus on addressing immediate and short-term support needs, resettlement, move-on and rents, these form the majority of tasks. The safety and wellbeing of families with children is paramount so a focus on Housing Management is a priority. There will be a high turnover of families moving in and out of the service so the accommodation will need to be turned around in a short time span. We are looking for reliable, friendly and enthusiastic candidates with an understanding of Safeguarding and a desire to work with often vulnerable families and children. Experience of working in an accommodation setting or similar environment is desirable and preferably, full training will be given. We welcome candidates with a background working with children and or young people. Car driver desirable, but not essential. Main job purpose To provide support to service users in a safe and maintained environment, by support planning, reviewing and assessing their individual risks and needs in order to prevent homelessness and secure and sustain chose accommodation. Main functions of this role; Create support plans and be responsible for assessing and evaluating the needs of service users, recording relevant information specific to the service users needs. Ensuring support plans are completed in a timely manner, with service users. To involve service users in all aspects of their support. Regularly update and review support plans. To risk assess service users and develop appropriate risk management strategies enabling service users to take responsibility and address challenges To ensure that tasks identified on the support plan are communicated to support workers and these tasks are clear, appropriate and agreed with the service user Provide support to service users and their children. General Responsibilities Provide a Housing Management function (Health and Safety Checks) collection of both ineligible and eligible rent, manage occupancy agreements, void and locality management. Ensure the accommodation provided is both safe and healthy. Develop and maintain good professional relationships with internal and external services/support networks. Carrying out welfare checks weekly to service users in our dispersed accommodation. Daily welfare checks are carried out for the 9 families in our hub accommodation Pelham Road.
