We are currently working with a local authority client in Cardiff seeking a Social Work Assistant to join their Community Mental Health Team for Older People (CMHTOP) on an agency basis. As a Social Work Assistant within the Community Mental Health Team for Older People, you will support the delivery of community mental health services to older adults. Working alongside social workers and health professionals, you will assist with assessments, support care and treatment plans, and help individuals achieve their outcomes through person-centred, strengths-based approaches that promote independence and recovery.
