We are seeking to recruit an experienced, hardworking and flexible Community Support Worker to join the Specialist Outreach Recovery Team. This post offers a challenging and exciting opportunity. You will be part of an enthusiastic, friendly, hardworking team delivering city-wide community-based interventions. We are a mental health recovery-focused team, working with clients who have a diagnosis of complex psychosis.

You will support registered members in delivering biopsychosocial interventions under the PCF framework. This role is integral to the developing recovery programme devised by the team. The role is diverse, and no two days are the same.


Main Duties

The role is an integral part of the multidisciplinary team consisting of consultant psychiatrists, psychologists, community mental health nurses, occupational therapists, substance use practitioners, social inclusion workers, assistant practitioners, and support workers. You may hold a caseload or work with others; interventions are planned jointly with the key worker. You will be part of the care planning process, assess risk and safeguarding, maintain accurate recording, and report concerns to the MDT.

You will receive appropriate training and any additional training identified as part of line‑management supervision.

You will work with individuals with complex needs. Candidates must have excellent communication skills, confidently liaise with other professionals, manage stress, reflect on practice with the team psychologist, and participate in caseload formulation sessions with the MDT.

All Livewell staff are expected to work across a 7‑day service.


Responsibilities

* Communicate verbal and written information sharing within the team and wider multidisciplinary team.
* Use telephone securely, maintaining confidentiality.
* Maintain accurate, timely paper and electronic records, securing them in accordance with policies and Data Protection Act.
* Demonstrate a positive attitude to mental health, attending to patients with respect and courtesy.
* Adhere to professional boundaries and maintain confidentiality.
* Collaborate in completing personalised care framework documentation under supervision.
* Communicate confidently with patients who do not speak English as first language.
* Collaborate with MDT members and other agencies; attend meetings, discuss patient care.
* Assist patients in social and therapeutic activities as planned by key worker.
* Plan, assess, contribute to development and delivery of group programmes; co‑facilitate recovery-focused groups with peer navigators, volunteers and other professionals.
* Update knowledge, skills, and practice; undertake mandatory training and Mental Health Act/PCF training.
* Plan community visits safely, considering cost and risk; perform rapid risk assessments.
* Liaise with other services to arrange appointments meeting patient needs and service demands.
* Plan own workload, complete White Board, implement packages of care, record visits electronically, submit e‑expenses.
* Navigate urban and rural environments; record physical observations (blood pressure, pulse, temperature) and report issues.
* Conduct home visits to multi-occupied buildings, including those without lifts; escort patients to appointments.
* Report risks, side effects of medication, and contribute to clinical case discussions.
* Support new staff and participate in team administration tasks.
* Adhere to all protocols, Code of Conduct for Healthcare Support Workers, Lone Worker policy, and National standards (CQC).
* Participate in research and development activities, including clinical audit.
* Work independently, seek guidance from qualified staff by telephone when needed.
* Carry out other duties commensurate with the position.


Person Specification


Qualifications

* Maths & English GCSE Grades 4 and above or equivalent.
* Commitment to further education.
* NVQ III or equivalent in Mental Health, or ability to demonstrate the skills, knowledge and ability to work to this level.
* Breakaway and/or conflict resolution skills.
* Knowledge of Care Programme or PCF.
* Approach to Mental Health Law.
* Understanding of Child and Adult Protection issues.
* Certificate in Mental Health level 3.


Knowledge

* Ability to travel independently and timely between Livewell Southwest sites and community locations, including areas not served by public transport.
* Practical life skills.
* Follow instruction from qualified practitioners.
* Confident talking to a variety of people.
* Self‑management skills (prioritisation, workload organisation).
* Self‑aware.
* Ability to manage own stress.
* Good telephone manner.
* Positive attitude to mental health.
* Belief in recovery.
* Enthusiastic and motivated.
* Aware of own boundaries and limitations.
* Able to work flexibly.
* Microsoft Outlook proficiency.
* Familiar with computer‑based recording systems.
* Clinical risk assessment skills.
* Support service users/carers to access resources provided by agencies.
* Support change process.
* Desire to develop self and services.
* Use own initiative.


Experience

* Experience of working and supporting people with mental health problems.
* Experience of team working.
* Experience in mental health work within either inpatient or community services.
* Multidisciplinary team working.
* Experience of helping people who are suffering from a mental health crisis.
* Experience of lone working.
