Your role and responsibilities We offer support and a chance to work in an interesting environment as part of a busy integrated team, with other health professionals. You will play a key part in promoting personalisation, independence, wellbeing and choice by providing a social care service for people. You will be expected to exercise a high level of autonomous professional judgement and risk evaluation, taking responsibility for the safety and well-being of people accessing adult and community services within a health environment. The role will include completing asset-based assessments/reviews, short term retirement plans, care and support plans and safeguarding plans in line with Supporting Lives, Connecting Communities and using a Signs of Safety and Solution Focused approach. You will need A strong working knowledge of relevant legislation, including The Care Act 2014 and The Mental Capacity Act 2005, and how these apply in practice. A solid understanding of Adult Social Care Services, including safeguarding procedures and the assessment, support planning, and care management process. Proven experience in applying a person-centred, enabling approach to support individuals in maximising their independence and wellbeing. Confidence in managing risk and safeguarding concerns, including the development of safety plans to reduce the risk of ongoing abuse. Experience of working collaboratively within integrated health and social care teams, including Social Workers, Occupational Therapists, Nurses, and other professionals.
