The Dementia Care Manager will be responsible for the overall management of the Dementia Community at Avery Healthcare, implementing and embedding the company\'s dementia care strategy and managing resources within a given budget. They will build caring relationships with residents and families, work closely with the Activities Team, and promote a culture that enables residents to maximize their capabilities and enhance their quality of life.

* Ensuring the plan of care is accurate and reflected in the care needs summary; residents and relatives identify needs and preferences, with information integrated into a person-centred care plan.
* Building caring relationships with residents and families, liaising with them sensitively and appropriately; acting as an advocate for residents\' best interests and managing conflicts when needed.
* Working closely with the Activities Team to develop meaningful activities for residents with dementia and supporting the care team\'s participation in these activities.
* Promoting a culture that enables residents to maximise their capabilities, optimise their independence, and enhance their quality of life.
