This post is available as an internal 12 month secondment due to the substantive postholder being seconded to another team to cover maternity leave. The postholder will undertake a key worker role, alongside an assessment and formulation role within the Living Well Service, which enables timely access to mental health services when people need it most and provides support for people who might not traditionally meet the threshold or criteria of existing mental health services. The Living Well Service is multidisciplinary and operates in partnership with Family Action (BAND), our third sector partner. The postholder will primarily cover South Bolton and will be based at Farnworth Town Hall. Experience of triaging and signposting referrals and liaising with multiple agencies and professionals is required. Experience and knowledge of community services and third sector organisations in Bolton will be beneficial.
Responsibilities
* Attend to triaging and signposting of referrals.
* Liaise with a range of agencies and professionals as part of a multidisciplinary team.
* Provide an assessment and formulation role within the Living Well Service.
* Cover South Bolton as required, with the designated base at Farnworth Town Hall.
Qualifications
* Experience of triaging and signposting referrals.
* Experience and ability to liaise with multiple agencies and professionals.
* Knowledge of community services and third sector organisations in Bolton is beneficial.
