We are looking for a highly motivated and committed Nurse Associate to join our Primary Care Network (PCN) and practice teams. In this key role, you will support the delivery of high quality primary care services and work closely with our multidisciplinary clinical team.
As a Nurse Associate, you will contribute to excellent patient care, help enhance the patient experience, and play an important part in the ongoing success and development of the PCN.
Main duties of the job
As a Nurse Associate within our PCN, you will:
* Deliver safe, evidence based, patient centred care across the lifespan
* Support clinical teams with assessment, planning, implementation and evaluation of care plans
* Conduct routine health checks, administer medications, and assist with minor procedures
* Perform diagnostic tests and assist with interpreting clinical information
* Provide health education and contribute to disease prevention initiatives
* Maintain accurate patient records and uphold data protection standards
* Collaborate with GPs, nurses, AHPs, and wider multidisciplinary teams
* Support referrals and ensure timely access to specialist services
* Participate in quality improvement and service development activities
* Engage in training and development to continually enhance your skills

Person Specification
* Valid nursing qualification and registration as a Nurse Associate with the appropriate regulatory body.
* Experience working within the NHS or a similar healthcare setting is desirable.
* Solid understanding of clinical procedures, protocols, and evidence-based practice guidelines.
* Excellent communication and interpersonal skills, with the ability to establish rapport with patients, families, and colleagues.
* Strong organizational skills and the ability to manage workload effectively in a fast paced environment.
* Flexibility to work rotating shifts, including days, evenings, nights, weekends, and public holidays as required.
* Proficient in using electronic health record systems and other relevant software applications.
* Commitment to maintaining professional standards, ethics, and practicing within the NHS framework.
* Experience working in a primary care or community healthcare setting.
* Additional training or certifications related to specialized areas of nursing practice.
