Overview
An exciting opportunity has arisen for a motivated Occupational Therapist to join the multidisciplinary community team at Dorking Community Hospital, part of the Surrey Downs Health and Care partnership hosted by Epsom and St Helier University Hospitals NHS Trust. The post holder will provide specialist occupational therapy assessment, treatment, and rehabilitation for patients with complex needs including frailty, multiple co-morbidities, admission avoidance, and discharge-to-assess pathways. Working across community and home settings, you will support individuals to maintain independence, improve functional ability, and achieve safe discharge from hospital services. You will manage your own clinical caseload, work autonomously while collaborating with the multidisciplinary team, and contribute to service development, supervision of junior staff and students, and quality improvement initiatives. This role is ideal for an Occupational Therapist looking to further develop their community rehabilitation skills, clinical leadership, and integrated care experience within a supportive and innovative healthcare partnership.
Responsibilities
Main duties of the job: To provide a comprehensive and coordinated range of specialist occupational therapy therapeutic and preventative services within community hospital, which maintain or improve people's health, independence and quality of life. Applying specialist knowledge and skills across the total range of complex health conditions, procedures and practices gained through practical experience, underpinned by theoretical knowledge. Care group includes frailty, complex co-morbidities, discharge to assess and admission avoidance. To assess and implement specialist treatment programmes as an autonomous practitioner, maintaining associated clinical records. To work unsupervised reporting back to the clinical lead on a regular basis and to work as part of the wider MDT. To participate in the day-to-day management of the service provision undertaking tasks as delegated by leadership, supporting new graduates and less experienced therapists, and non-registered staff members on a daily basis.
