Job Summary
We are looking to expand our team with multi-disciplinary professionals to work alongside the GP Triage team and to provide a safe and efficient day‑to‑day service for patients with acute conditions. We need a prescribing clinician with experience in minor illness.
Typical Responsibilities
* Assess patients with minor illness presentations
* Take clinical history and perform physical examinations
* Diagnose common acute conditions
* Prescribe medications (if Independent Prescriber)
* Provide treatment plans and safety-netting advice
* Order and interpret basic investigations (e.g., urine dip, throat swabs)
* Refer to GP, urgent care, or hospital when necessary
* Document consultations in clinical systems (EMIS)

Main Duties of the Job
Minor Illness Nurse or Paramedic in General Practice will assess, diagnose, and treat patients presenting with non‑life‑threatening acute conditions. The role includes independent clinical decision‑making and, when qualified, independent prescribing.
The clinics will be daily and may include home visiting or specialty clinics (respiratory or paediatrics) depending on the successful candidate’s skills and experience.
Primary duties include:
* Asses, diagnose, plan, implement and evaluate treatment/interventions for patients presenting with an undifferentiated diagnosis focusing on minor ailments and/or minor injuries
* Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs
* Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long‑term condition (as appropriate)
* Diagnose and manage both acute and chronic conditions, integrating both drug‑ and non‑drug‐based treatment methods into a management plan
* Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence‑based practice and national and practice protocols, and within scope of practice
* Work with patients to support compliance with and adherence to prescribed treatments
* Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
* Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self‑care
* Assess, identify and refer patients presenting with mental health needs in accordance with national and local guidelines
* Meet the needs of patients presenting for opportunistic wound care
* Undertake minor surgery as appropriate to competence
