Job responsibilities
* Lead the day‑to‑day operational delivery of Patient Flow and Discharge, deputising for the Deputy Director of Nursing and AHPs when appropriate.
* Ensure patients receive safe, efficient, high‑quality, and cost‑effective care across all responsibilities and take an active role in the Trust’s corporate objectives, leading Trust‑wide initiatives/developments and working with system partners to support safe and timely discharge.
* Design services to meet the evolving needs of the population in line with current government initiatives, Trust and local targets.
* Act as clinical lead for the service, being a registrant and providing expert clinical expertise when dealing with stakeholders.
* Develop, implement and evaluate the clinical elements of service redesign, acting as an expert resource to key stakeholders, managerial staff, patients and carers.
* Hold line‑management responsibility for the Care Group, Patient Flow and Discharge team, ensuring personal development needs are met.
* Ensure the Patient Flow Team provides 24/7 safe patient placement, appropriate inpatient bed allocation, professional and clinical leadership for all clinical areas (in and out of hours), and leads the operational out‑of‑hours response to any critical or major incidents.
* Ensure the discharge team provides nursing leadership to safe patient discharge, expert knowledge of complex discharge pathways, effective communication with partner organisations, and serves as the point of contact for staff, patients, relatives and other organisations.
* Co‑ordinate major and critical incidents, ensuring comprehensive site service provision.
Person Specification
Qualifications
* Professional registration (NMC registered nurse, HCPC registered therapist, OPD or HCS)
* Degree or equivalent professional qualification
* Postgraduate qualification at Master’s degree level or equivalent
* Leadership certificate
* Evidence of recent and relevant professional CPD
* Evidence of further study in areas such as leadership, project management, or related fields
Experience
* Experience of working with multi‑disciplinary teams and achieving key service improvement outcomes in a challenging environment
* Experience of budgetary management
* Demonstrated operational management experience in an Acute Hospital & Community environment
* Experience in managing organisational change
* Teaching experience
* Experience of Practice Development
* Experience of developing policies and protocols
* Experience of managing a range of staff groups / specialities
* On‑call manager or equivalent experience
* Experience of developing joint and collaborative working with local partners including other NHS Trusts, voluntary organisations, and the Local Authority
* Involvement in major projects and experience of professional leadership of services
* Experience of managing change within a health care environment
