What the Job Involves This role involves providing and coordinating support for care leavers to achieve their pathway plan goals. You will offer advice and support, participate in assessments and planning, design interventions, ensure the implementation of pathway plans, and coordinate services. Keeping up-to-date client records and actively challenging discriminatory behavior are key parts of the job. You are also responsible for reporting any concerns regarding a young person's welfare or safety.
