In this role, you will support our triage decision-making process, produce care plans, and work directly with individuals in the Neighborhood East community of Somerset.

Qualifications and Experiences:
To be eligible for this role, you must have Social Work England registration and be eligible to work in the UK. Additionally, you need to hold a full UK driving licence and possess post-qualified experience. A degree or equivalent qualification in Social Work is also required.
