Job overview

Healthier South Wirral Primary Care Network (PCN) and CWP’s Children Young People and Families mental health services (CYPMHS) have worked together to enhance access to mental health support.

We aim to create a more integrated community-based model of mental health care supporting both primary and secondary care services, facilitating early intervention and seamless transitions.

Primary Care Networks (PCNs) are groups of GP practices working together locally. The Healthier South Wirral Care Network comprises 6 GP Practices caring for over 49,000 patients.

We seek an experienced Mental Health Practitioner to join the GP Multidisciplinary Team in the South Wirral PCN. This role supports NHS 10-year Plan initiatives, keeping care in the community, and aligning with I-Thrive principles.

We are recruiting 2 part-time posts, each 0.70 FTE (26.25 hours).


Main duties of the job

The role includes conducting assessments, delivering brief interventions, signposting to community resources, supporting neurodevelopmental needs, facilitating transitions, building community networks, and providing training and advice to GPs and the multidisciplinary team.

You will be line managed by the CWP Band 7 Access Team Manager and will have access to CWP training and support. The role suits autonomous individuals committed to collaborative mental health care improvements.

* LD CYPMHS teams
* Third sector pathway partners
* Community Paediatrics

Qualifications

* Registered Mental Health Professional (e.g., RMN, Occupational Therapist)
* Degree-level education or equivalent experience
* Masters qualification or relevant postgraduate courses
* Recent CPD evidence
* Teaching qualification or significant experience
* Postgraduate psychological therapy qualification or equivalent


Knowledge and Expertise

* Excellent communication skills for working with staff, patients, carers, and in reports
* Ability to challenge practices and implement evidence-based changes
* Understanding of local and national policies
* Experience in clinical audit and facilitating staff development
* Leadership skills and clinical expertise
* Proficient IT skills


Experience

* Clinical supervision experience
* Development of professional roles
* Managing projects and implementing change
* Knowledge of clinical and corporate governance
