Overview
Join to apply for the Support Time Recovery Worker role at Kent and Medway NHS & Social Care Partnership Trust. You will join the Review and Resettlement Team and support individuals to transition from hospital to community living across a wide geographical area with personalised, recovery-focused care.
Responsibilities
* Work 1:1 or 2:1 with service users to promote independence and wellbeing.
* Support people to develop daily living skills, build community connections, and achieve recovery goals.
* Collaborate with a multidisciplinary team to deliver safe, high-quality care.
* Travel to meet service users admitted out-of-area and support them in their home communities during transition.
Support and Discharge Planning
You will support the Complex Care Coordinator with the discharge planning process for patients admitted out of area, funded by the Mental OATS panel. The role involves working as a member of the Review and Resettlement Team and actively contributing to the discharge planning process to improve the patient journey and minimise delays. Under the supervision of the Complex Care Coordinator, you will collaborate with other professionals toward effective discharge planning and help patients settle in their new environment for a minimum period of 3 months, facilitating transition back to Community Services.
Requirements
We are looking for someone with experience in mental health or social care, excellent interpersonal skills, and the ability to work flexibly and independently. A commitment to equality, diversity, and inclusion is essential.
