Overview
GP with a Specialist Interest in Frailty Virtual ward and Urgent care Response. We have an exciting new role for a GP to join our interdisciplinary, integrated community team providing more acute care for patients in their own homes. This is a sessional role with flexible number of sessions to suit the successful applicant, and administration time is included. The role is a new position for the department and focuses on delivering acute care for patients safely and conveniently in their own homes as part of national frailty service design. The successful applicant will primarily work within our developing virtual ward model, supporting GPs to access the right care at the right time for their patients and to utilise our expanding community-based services as an alternative to hospital admission. We work closely with the ARCH team (Acute Response and Rehabilitation in the Community and Hospital), including occupational therapists, physiotherapists, nurses and multidisciplinary assistants. A dedicated senior frailty pharmacist is attached to the team. The role involves working as part of a multidisciplinary team to support and enhance a comprehensive Geriatric Service within the Trust, uphold high clinical standards, and contribute to teaching, training and assessment of UCR staff and other team members. It also includes participation in departmental and Trust matters concerning Clinical Governance and audit, and active collaboration with Ripon GP practices to increase uptake of services to support Care Home residents, along with responsibility for continuing medical education (CME).

Organisation and Training
HDFT provides a range of community services in Harrogate and the local area as well as across North Yorkshire. Our dedicated and experienced staff, based in the communities they serve, offer expertise across disciplines and work closely with primary care, acute hospitals, social care, mental health and voluntary sector providers. The Trust is a provider of undergraduate medical education for the University of Leeds and has a thriving research community. We have developed Frailty education faculty, including the appointment of frailty practice educators, enabling frailty training for all with options including e-learning, face-to-face study days and Enhanced care of Older People (EnCOP) training. The successful candidate will be able to access this training and there is opportunity to contribute to the faculty.
