Job Description: Trident Healthcare Solutions is seeking an experienced Band 6 Occupational therapist for a locum assignment based within the Intermediate Care Team at an NHS Trust in Hull. The successful candidate will independently manage a caseload of adult patients.
Responsibilities
* Manage a caseload of intermediate care patients
* Complete assessments, treatment plans, discharge planning and onward referrals
* Ideally hospital discharge./ Intermediate care/ discharge to assess experience
* Maintain accurate and timely patient records
Skills and Experience Required
* Experienced working within the NHS (or similar)
* SystmOne experience desirable
* Excellent written and verbal communication skills, including phone and email
Essential Requirements
* Candidates must have NHS experience
* Candidates must hold HCPC registration
* Candidates must hold the right to work in the UK
* Must have car as potential for travelling to other sites and patients' homes
