Community Mental Health Nurse - Named Worker Role

The South Community Mental Health Service is undergoing an exciting transformation with additional resource allocation to provide ongoing intensive support to the population of Sheffield, this includes working towards a joint approach with Primary Care and the Voluntary Sector, reviewing the CMHT Service Criteria and working in discreet small teams into local communities leading to a high quality care standard.

We are looking to recruit Band 6 Nurse to the South CMHT. A number of our service users require a man as their named worker due to the nature of the service provided including trauma and risk history. We intend to therefore appoint a man to this post as an occupational requirement for this particular post because we do not have any men in nursing roles in this service.

The successful candidate will be supported with their professional and leadership development through dedicated supervision with highly experienced and motivated team managers, where learning opportunities will be supported and encouraged. From the multi-disciplinary team, we offer regular CPD sessions and candidates are encouraged to bring their own interests to the team to share and develop their specific interests in mental health. Please note this is a qualified Nursing Vacancy.


Main duties of the job

You will need to possess excellent communication skills and be able to work independently and as part of the wider team in order to assess, plan and evaluate care. You should have a positive approach to meeting the needs of our service users and be motivated, empowered and enthusiastic to both learn and share.

You will also be expected to work as part of the duty system which operates within the general working hours of 09:00 to 17:00 Monday to Friday on a rotational basis. This "mini team" is supported by other clinical professionals and management.

Job responsibilities

The post holder will work within a multi-disciplinary service and core responsibilities will include:

* Acting as a Named Worker
* Comprehensive assessment of new referrals
* Carrying a varied caseload defined by need
* Contributing to a duty system on a rota basis
* Delivering supervision to more junior members of staff
* Contribute to Multidisciplinary service development


Clinical

* Triage and determine urgency of new referrals to the service
* Carry out comprehensive assessments of an individual's health and social care needs in a variety of settings and agree the most appropriate course of action. This includes first presentation to service.
* To assess, evaluate and manage clinical risk
* To formulate a plan of care based on identified need
* Deliver evidence based interventions as appropriate
* Review effectiveness of care plan at regular intervals
* To work within safeguarding processes, including where required to act as safeguarding investigator / manager.
* To work collaboratively with service users, carers and partner agencies
* To maintain computer held care records in an accurate and timely fashion
* Undertake all work in a culturally sensitive manner
* Act in accordance with Care Management System, Acute and Schedule Care Pathways as appropriate.
* Develop effective working relationships with Primary Care colleagues

Person Specification


knoweledge and skills

* Working knowledge and understanding of Care Programme Approach & Care Management
* Knowledge and understanding of appropriate therapeutic approaches and engagement strategies
* Knowledge and understanding of PDR and supervision processes as a supervisee
* Demonstrable risk assessment and risk management skills with evidence of positive risk taking
* Additional skills /training in psychosocial interventions for people with SMI, CBTp and family interventions
* Relevant Health or Social Care professional qualification - Nursing or Social Work


Training and qualifications

* Relevant Health or Social Care professional qualification - Nursing or Social Work
* Current registration with NMC, HCPC
* Evidence of continued professional development
* Supervision training


Experience

* Experience of multi-disciplinary team working
* Extensive experience in mental health in both inpatient and community settings
* Experience of managing/supervising others
* Experience of working in a recovery focussed way
* Experience of working with adults in acute/crisis/relapse presentation
* Experience in a Community Mental Health Team/Assertive Outreach team
