Role Overview
There is an exciting opportunity for a Band 6 Specialist Nurse within the Community Palliative Care Team. The Community Palliative Care Team is a dynamic multi‑disciplinary team who provide specialist palliative care services to Oxfordshire and South Northamptonshire. This includes home visiting, outpatient, Attend Anywhere clinics and telephone advice.
Responsibilities
* Provide specialist palliative care services across a 7‑day service, including pro‑active support, telephone support and a Virtual Ward.
* Work weekend and extended hours as required.
* Conduct patient‑focused expert clinical assessment and judgement.
* Develop autonomous and multi‑disciplinary team working.
Qualifications and Requirements
* Band 6 Specialist Nurse grade.
* Palliative or end‑of‑life care experience essential.
* Excellent communication skills in supporting patients and their families.
* Strong leadership qualities.
* Full driving licence and car ownership.
* Ability to work anywhere within Oxfordshire and South Northamptonshire.
