Social Care Reablement Occupational Therapist
We are looking to recruit a self-motivated Band 6 Occupational Therapist with excellent goal-setting and communication skills to join our friendly, supportive, and experienced team.
You will be the sole Occupational Therapist supported by an experienced Occupational Therapy Assistant covering East Devon.

Main duties of the job
In this role, you will:
* Use your core occupational therapy skills to assess the needs of adults with a wide range of disabilities and health conditions.
* Enable individuals to maximise their independence and potential, while increasing safety and reducing risk to themselves and their carers.
* Hold and manage a caseload including lone working, providing a person-centred service through clinical assessment within the context of local government policy and legislation.
* Deliver interventions guided by the Care Act 2014 eligibility criteria and a strengths-based approach. These may include signposting or advice, provision of equipment, minor and major adaptations, support with rehousing, recommendations for care packages, or referrals to other teams.
* Screen complex referrals, support team leaders through joint visits and peer supervision, and contribute to the development and delivery of updated training for our team of support workers.
* Act as an autonomous practitioner working across different community settings. Be responsible for managing risk and delivering safe, person-centred interventions. Be able to solve problems in partnership with the individuals you support.

The Eastern Social Care Reablement team is based at Great Moor House in Exeter, supporting individuals across East and Mid Devon, including rural communities, towns, and the city of Exeter. We are a dynamic and supportive team made up of Occupational Therapists, Reablement Team Leaders, and Reablement Support Workers, working collaboratively to promote independence and recovery.
We work closely with NHS colleagues, other adult social care teams, housing providers, and community services to deliver timely, person-centred interventions. Our assessments are carried out face-to-face in people's homes, as well as virtually or by telephone, depending on the needs and preferences of the individual.
Our focus is on short-term, strengths-based interventions that empower people to regain skills and confidence following illness, injury, or changes in circumstances, helping them to live as independently as possible.
