Overview
You will be responsible for delivering quality care to a designated caseload of people in the place that they call home, following a care plan devised by the community nursing staff you will be working alongside. You will be asked to feedback concise information in a daily handover to ensure changing needs are met and risks escalated.
Responsibilities
* Deliver quality care to a designated caseload in the home setting.
* Follow a care plan devised by the community nursing staff you will be working alongside.
* Provide concise daily handover feedback to ensure changing needs are met and risks escalated.
* Be flexible and assist colleagues to cover service needs across the wider integrated team.
* Support service delivery during operating hours 08:00–21:00 with rotational shifts including weekends, evenings and Bank Holidays.

* Ability to work across the wider integrated team and flexibility to cover service needs
* Ability to provide care to adults over 18 living in Bedfordshire
