An exciting opportunity for an experienced Social Work Practitioner to join Salford Adult Social Care. We support people to live well and be safe at home, promote wellbeing through strength‑based conversations, and work closely with other organisations to connect people with local communities.
Qualifications
* Hold a degree in social work.
* Registered with Social Work England.
* Evidence of continuing professional development.
* Strong understanding of the Care Act, Mental Capacity Act and Safeguarding Adults.
Responsibilities
* Complete strengths‑based assessments and promote well‑being by utilising resources from social care, health and other voluntary or statutory agencies.
* Undertake initial assessments and proactively work with complex and legal issues, crisis planning, provide information, advice, signposting, and transfer to other social work teams or services as required.
* Identify need for further assessment under sections 9 and 10 of the Care Act, and transfer for assessment. Complete support planning and reviews.
* Conduct safeguarding functions: gather initial information, risk assess, safety plan and determine whether Safeguarding Adult Enquiries under section 42 of the Care Act are required.
* Manage competing demands, prioritise tasks effectively, be confident decision‑makers and stay calm under pressure.
