We are redesigning our community services model and implementing a new Intermediate Care Team, creating an exciting opportunity for nurses to deliver urgent community response services (operating from 8am to 9pm, 7 days a week). This role is crucial in providing essential support and care to individuals, focusing on promoting independence, dignity, and well-being. It offers an excellent opportunity for career progression in nursing.
The objective is to facilitate safe and timely discharge from inpatient care for patients needing ongoing care or to prevent admission, through interventions manageable at home.
The post holder will collaborate closely with medical and therapy colleagues within a multidisciplinary team for adults as part of the Intermediate Care (Hospital@Home) Service.
You will support the Team Leader and Deputy in developing and implementing the service, including facilitating integrated working with Local Authorities and third-sector organizations to support individuals in their homes where appropriate.
Responsibilities include managing patients within various settings, providing clinical assessments, and developing treatment plans. Delegated responsibilities from Team Leaders include timely patient management and care planning.
Welsh Skills (Desirable): Speaking and listening skills at Level 3 (CEFR B2 or above) are advantageous but not essential. Basic conversations about health are sufficient.
