Thepaediatric pharmacy team at Leeds Childrens Hospital have an excitingopportunity to join our dynamic, friendly, and forward-thinking team as oneof the Advanced Clinical Pharmacists for Paediatric and Neonatal Nutrition.
Workingwith our established Paediatric and Neonatal Nutrition ACP you will supportthe growing nutrition service and contribute to the delivery of the provisionof pharmaceutical care across the paediatric team.
Tobe successful you will be an enthusiastic dynamic and motivated pharmacistwho can lead and develop pharmacy services within the paediatric nutritionteam. The nutrition service sees Children from Leeds and around Yorkshirewith a wide range of disorders, such as intestinal failure, feedingdifficulties, poor growth gastroesophageal reflux, gut dysmotility, coeliacdisease, Crohns disease and ulcerative colitis.
Theparenteral nutrition service works in close partnership with our asepticsunit and home PN suppliers, and strong working relationships with these teamsare essential to the effective delivery of care. You will have a passion formaking a difference to patient care and be committed to workingcollaboratively with both pharmacy colleagues and the multidisciplinary team.
Paediatricnutrition experience is preferred but not essential and you would besupported by the team to develop knowledge in the area.
Main duties of the job
Thisis a shared role, introduced in response to increasing demand in bothinpatient and outpatient parenteral nutrition services. You will workalongside the current Advanced Clinical Pharmacist in post. While there willbe considerable overlap in day-to-day responsibilities, elements of the rolewill be divided based on each individuals experience and areas of expertise.Shared ownership will be key, with each ACP potentially taking the lead ondifferent aspects of the service to ensure continuity, resilience, andconsistent high-quality care across the pathway.
Youwill share leading the specialised clinical pharmacy service to patientswithin paediatric nutrition, providing expert clinical pharmacy input andoversight, organising, prescribing and advising on patients requiringparenteral nutrition within the Leeds Childrens Hospital alongside ourpaediatric home parenteral nutrition patients. The post will give you the opportunity tobe an integral part of the MDT. You will monitor and report on clinical andfinancial governance of medicines, advise on potential improvements and leadon these where appropriate. You will be responsible for recording andinterpreting blood results and adjusting formulations to meet both clinicalneed and formulation compounding stability.

2. JOB PURPOSE
Todeliver and develop specialist clinical pharmacy services to the LeedsChildrens Hospital within the Trust, through provision of.
3. JOB DIMENSIONS

Reporting to the Clinical PharmacyTeam Leader the post holder will be responsible for the day today clinicalpharmacy service to inpatients ward at LTH. The post holder will communicateacross multi-disciplinary teams providing specialist advice on appropriatenessof medicines prescribed, across paediatric clinical specialities.
Ensure medicines are usedappropriately, safely, and cost effectively in accordance with Trust policy,standard operating procedures, and medicines legislation.
Monitors use of medicines withininpatient prescribing. This includes recording of significant clinicalinterventions, risk management and some basic financial aspects.
Support other members of the teamwith specialist advice regarding the development and implementation oftreatment protocols and guidelines for use of drugs within the clinicalareas.
Responsible for trainingpharmacists, and other health care professionals, on aspects of drug usewithin the outpatient clinics.
Provides service cover in thedispensary, aseptics with ward cover when and if required. Participates inthe departments weekend and Bank Holiday rotas and where appropriate, out ofhours services.

5. KNOWLEDGE, SKILLS AND EXPERIENCEREQUIRED
Master of Pharmacy degree (4years), or equivalent qualification, plus 1 year pre-registration training.
Registrant of the GeneralPharmaceutical Council.
Ability to demonstrate competencyin all domains of the General Level Framework (GLF)
Substantial post-registrationexperience as practising pharmacist.
Experience of dealing with complexclinical issues, that require the analysis of a variety of sources ofinformation, these may not be in agreement when reaching an appropriateresponse. The pharmacist will have experience in negotiating with and persuadingother members of the multidisciplinary clinical team, or patients/clients, toagree on a course of action
Experience of dealing with clinicaland non-clinical staff, typically at junior grades, but occasionally athigher consultant and managerial levels.
Ability to comprehensively counselpatients on use or sales of medication ensuring appropriate advice issupported by written information using patient information leaflets.

9. PRINCIPAL DUTIES & AREAS OFRESPONSIBILITY
Provide specialist clinicalpharmacy service to patients and other members of the multi-disciplinary teamover a wide range of specialities. This will include:
Individual prescription review tooptimise therapy.
Utilising patient medication recordsto support review of medicines suitable for use.
Advice on dosage and side effects.
Advise on safe administration ofmedicines of both prescribed and over the counter sales.
Responsibility for medicines riskmanagement in a variety of clinical areas, including investigating clinicalincidents, recording significant clinical interventions/near misses, andensuring compliance with medicines legislation, and local policies.
Assist in the introduction of newdrugs, including unlicensed medicines, into the clinical specialty.
Organisation and planning of ownworkload on an on-going basis e.g. ward cover, training and CPD, audit,project work, attendance of meetings etc.
Supervise, teach and assess juniorpharmacy staff completing training in outpatient dispensaries. This couldinvolve: -
Providing support for studenttechnicians.
Being a named tutor for apre-registration pharmacist.
Being a mentor to juniorpharmacists.
Participate in any medicinesrelated clinical trials relating to outpatient services speciality.
Propose and carry out clinicalpharmacy practice research within outpatient areas and participate in anylarger pharmacy research and audit.

Involvement in reviewing andimplementing clinical policies and procedures relating to outpatientservices.
Advise on over the counter sales ofmedicines to customers and patients.

11. SPECIAL WORKING CONDITIONS
i) PHYSICAL EFFORT:
Thepost requires a combination of sitting, standing, and walking with littlerequirement for physical effort, excepting an occasional need to movepharmacy boxes, fluids, enteral nutrition packs. These typically weighapproximately 10kg per box.

ii) MENTAL EFFORT:
Thepost required frequent amounts of concentration for clinical and accuracychecking, reviewing prescriptions, performing complicated calculations etc.Post holders will frequently be interrupted by urgent or non-urgent clinicalenquiries, received verbally, or by telephone or bleep, at any time,including during breaks.

iii)EMOTIONAL EFFORT:
