Prestige Nursing East Lancashire is looking for an additional Care Planner to join our office team in planning, organising and rostering support for individuals utilising our services within their own homes and care settings.
We are offering a competitive salary package of £25,740 to £30,185 dependant on experience to be agreed with the successful candidate.
Having secured a significant amount of new work opportunities, we need compassionate, committed, caring staff to join our team and support our service users across the region.
Experience of planning and rostering is preferred but not essential, however the candidate should have a flexible, enthusiastic, can-do attitude to join the team working from our Blackburn office.
You will be assisting and supporting the branch leadership team in ensuring key business and compliance targets are met. Primary responsibilities will include completing weekly rosters, covering any immediate sickness or absence amongst the community care/support teams, and handling calls that are made to our offices by responding to them directly or passing on to the appropriate colleague.
The role of Care Planner is key to ensuring our community care/support teams are utilised effectively, meeting the specific needs and preferences of service users whilst maintaining defined quality, compliance and safety standards.
We endeavour to keep you working as close to home as possible, but you will require access to your own vehicle and a full UK driving licence.
The Role:
Tasks will include:
1. Completing rosters using Webroster
2. Accepting, allocating and processing referrals for new care packages
3. Accepting and processing changes and amendments to existing care packages
4. Understanding, organising and recording the availability of all new care workers
5. Identifying growth opportunities
6. Forming strong relationships with all stakeholders
7. Working closely and communicating with other members of the team, professional bodies and family members to ensure the best possible outcomes for all stakeholders
Success in the role will be measured by:
8. Ensuring member utilisation targets are met
9. Assessing branch performance against weekly, monthly, and annual targets
10. Reviewing effective shift rostering, including forward planning, in ensuring all service users have suitably allocated care and support.
