Band 4 - Integrated Neighbourhood Team Coordinator

At the Birmingham and Solihull Community Care Collaborative, we are looking for reliable, flexible, and proactive individuals to join our Integrated Neighbourhood Teams as Coordinators.
In this role, you will work with professionals across health, social care, and the voluntary sector to deliver more joined‑up, personalised care for people within our communities. You will play a key part in improving communication, supporting meetings, coordinating actions, and ensuring that information is shared effectively to achieve better outcomes for citizens.
You will be aligned to a specific Integrated Neighbourhood Team and are expected to provide cover for colleagues when required. Flexibility to travel across sites for meetings and events is required.
Responsibilities
* Act as the central point of contact and navigator between your designated Primary Care Network (PCN) and the Integrated Neighbourhood Team (INT) multi‑disciplinary team (MDT), establishing effective communication processes across primary care, community services, mental health services, and the voluntary/third sector.
* Work collaboratively with the INT MDT and liaise with key workers to manage patient information that supports the MDT’s triage and MDT meetings, capturing outcomes and evaluation.
* Facilitate the decision‑making process within the INT MDT.
* Travel independently across the Trust to attend meetings and events as required.
Qualifications / Training
* Educated to GCSE level or equivalent with grade C or above in English and maths.
* ECDL or equivalent IT qualification / experience.
* IT systems and project management experience equivalent to a diploma.
* Evidence of continuous personal development.
Experience
* Experience of administrative procedures.
* Experience of working in a customer‑facing environment, resolving queries, and using initiative to ensure effective working relationships across services.
* Previous NHS experience or experience of working in a healthcare / social care setting.
Skills / Knowledge
* Competent in the use of IT software systems (including Microsoft Office) and handling sensitive personal identifiable data.
* Demonstrable evidence of problem solving and resolving issues as they arise in a fast‑changing environment.
* Ability to communicate clearly and sensitively with a wide range of people, including external partner organisations, community teams, and patients and their families.
* Excellent written communication skills; able to write clearly and concisely and produce reports and keep accurate records.
* Ability to induct new staff into a team, providing training to new colleagues.
* Adaptable and flexible approach to work with the ability to prioritise workload.
* Understanding of the importance of confidentiality and the Data Protection Act.
* Self‑motivated with the ability to motivate others.
* Ability to work autonomously without direct supervision.
* Ability to prioritise conflicting tasks and demands.
* Willingness to encourage and drive innovation.
* Ability to set high standards for self and others to deliver service priorities.
Other job requirements
* Able to travel independently across the Trust as service needs demand.
