Job Title: Social Worker – Adult Constituency team
Location: Birmingham
Rate: £31.48 per hour
An exciting job opportunity has recently become available in Birmingham. They are seeking a new team member for their Adult Constituency teams. This role offers a total of £31.48 per hour for the contract duration.
What will your responsibilities be?
* Support vulnerable adults in the community by promoting independence, well-being, and social inclusion.
* Collaborate with individuals, families, and professionals to achieve positive outcomes through compassionate assessment and support planning.
* Conduct comprehensive assessments and develop personalized support plans.
* Coordinate services with health professionals, care providers, and community resources.
* Identify and respond to safeguarding concerns, assess risks, and ensure legal compliance and safety.
* Provide emotional support, counselling, and advocacy to clients.
* Assist clients in accessing services, benefits, and housing, and facilitate group sessions and workshops.
* Liaise with healthcare professionals, housing officers, police, and attend multidisciplinary meetings.
* Maintain accurate case records and ensure compliance with data protection and confidentiality guidelines.
Qualifications and Experience
* Social Work England registration and eligibility to work in the UK.
* Full UK license and post-qualification experience.
* Degree or equivalent in Social Work.
* Experience in Adult care with knowledge of legislation such as the Care Act 2014 and Mental Capacity Act 2005.
