We are looking for an experienced and enthusiastic nurse to join the innovative Specialist Palliative Care Team (SPCT), working in the community care team. To provide expert, patient-centered palliative care within the community, supporting individuals with life-limiting illness.
The post holder will provide expert clinical leadership and education across palliative and end of life care services within our Trust.
You will have a defined caseload, setting and maintaining high standards of care through advanced assessment, planning, implementation, and evaluation. Acting as a clinical expert, you will support patients, families, and colleagues to deliver compassionate, evidence-based, person-centred care.
Working as part of a Multi-Disciplinary SPCT Team, you will lead on the development and implementation of our education strategy across the organisation.
You will deputise for the SPCT Leader as required and ensure effective use of resources while promoting a culture of continuous learning and improvement across all care settings.

Main duties of the job
1. Provide expert clinical leadership and advanced palliative care to patients with complex needs across acute and community settings. Act as a role model and source of specialist advice to nursing and multidisciplinary teams.
2. Complete advanced assessments, develop and evaluate individualised care plans, and use non-medical prescribing (where appropriate) to ensure timely and effective interventions.
3. Lead the development, delivery, and evaluation of a Trust-wide education strategy for palliative and end of life care. Design and facilitate learning programmes for all staff groups and contribute to post-graduate teaching and training.
4. Work with SPCT to develop nurse-led initiatives, implement evidence-based practice, and lead on quality improvement projects that enhance patient outcomes and experience.
5. Provide day-to-day leadership within the SPCT and deputise for the Team Leader when required. Contribute to workforce development, supervision, and appraisal of staff, and support service planning and resource management.
6. Participate in and lead clinical audits, contribute to the research agenda within specialist palliative care, and evaluate outcomes to inform policy and practice improvements.
7. Communicate complex and sensitive information with patients, families, and professionals. Foster strong multidisciplinary and cross-sector partnerships to ensure seamless, person-centred care across all settings.
