This is an exciting opportunity for an Advanced Nurse Practitioner to support patients with urgent, on-the-day care needs in the local community. This fixed-term role for 6 months (secondment considered) to cover maternity leave will support GPs and the multidisciplinary team to deliver safe, effective, and person-centred care in a primary care setting. Successful applicants may work in GP practices, visit care home residents, or attend to patients in their homes. The role also emphasizes the Four Pillars of Advanced Practice, supporting development in education, mentoring, audit, leadership, and quality improvement.
Applicants must currently work in an Advanced Practice role and have completed MSC (Level 11) Advanced Practice studies to PG Dip level or have equivalent experience.
Excellent communication skills and the ability to work as part of a wider health and social care team are essential. The role requires autonomous working, clinical judgment, and decision-making to support the nursing workforce within practices. This is a chance to join a dynamic team committed to quality care.
