What is our PCN Project?
This project is a new and exciting opportunity to help reform and influence Derbyshire’s end of life care, with a strong focus on the link between identification, deprivation and admission. Our PCN Project is taking a data driven approach and focuses on four identified PCNs with low identification and high deprivation at end of life
4. Chesterfield & Dronfield PCN
5. North Derbyshire PCN
6. North Hardwick Bolsover PCN
7. South Hardwick PCN
An exciting aspect of this project is that, with a strong focus on admission avoidance and early identification, we will be embedding Marie Curie nurses into Chesterfield A&E, empowering that nurse with taking a “why not home?” approach and striving to ensure preferred place of death is an upheld choice.
What is the Role:
We are looking for an experienced Palliative Clinical Nurse Specialist/ Advanced Nurse Practitioner to join this team, you'll be instrumental in developing and shaping this service. The key aim will be to improve identification of patients with palliative and end of life care needs through advanced clinical assessment and multidisciplinary working.
We are looking for an individual with a confident and competent clinical and person-centred approach who can utilise their skills and experience to have an impact on increasing access to palliative/end of life care and support for those in the most deprived areas, enhancing the health and well-being of individuals and their carers. We seek a dedicated individual to join us in making a positive impact.
In this role, you will work closely with the Senior Management Team, senior nursing staff, key members of the multi-disciplinary team, service users, and colleagues within the Network and the Charity. This role will exemplify person-centred leadership, clinical excellence, and effective collaboration, ensuring that Marie Curie meets its strategic goals and positively influences the care delivered.
Key Responsibilities include:
8. Develop nursing staff and multidisciplinary team members through support, advice, education, reflective practice, and clinical supervision.
9. Provide holistic specialist palliative care advice to patients and their families.
10. Conduct Nurse Led Clinics in the Outpatient Unit and collaborate with staff on the Inpatient Unit.
11. Lead and innovate nursing practice within the speciality throughout the Hospice.
12. Develop policies and evidence-based practices as appropriate.
13. Contribute to the development and smooth running of the service.
Job Structure : Permanent; Full Time 37.5 Hours/Week
Shifts : Day Shifts 9:00am – 5:00pm Monday to Friday
Salary :Marie Curie Clinical Pay Scale Band 7 (Aligned with AFC) £47,810 - £54,710 Per Annum, Plus Enhancements
Based : Chesterfield Royal Hospital, A&E department
What we’re looking for:
14. NMC Registered Nurse with a Clinical Nurse Specialist qualification/Advanced Nurse Practitioner qualification.
15. Good knowledge of professional accountability and responsibility and the limitations of the role.
16. Recent/current experience in Specialist Palliative Care and Caseload Management.
17. Ability to work collaboratively in a challenging environment
18. Strong communication and training skills
19. Commitment to continuous improvement and sharing best practices
20. Quality improvement and service implementation experience desirable.
