What you will do: Conduct comprehensive assessments of clients referred to the Community Mental Health Team, with a focus on delivering person-centred and holistic care. Develop and implement care plans tailor-made to meet each client's unique needs, collaborating with multi-disciplinary teams. support and intervention for individuals experiencing mental health crises, ensuring they receive timely and appropriate care. Liaise with families, caregivers, and external agencies to ensure coordinated and cohesive support networks for clients. Maintain accurate and up-to-date records, ensuring all documentation complies with legal requirements and best practice standards. To be successful in this role, you will need 5 years of post-qualification experience, with evidence that the Assessed and Supported Year in Employment (ASYE) was completed within two years of qualifying. You must have at least 2 years' experience as a Social Worker (non-agency) specifically within a mental health team, with particular value placed on experience in an Older Adults Community Mental Health Team. A driver's licence is required, and you must be able to start within 4 weeks with 3 years of sound references and no gaps in employment.
