Thisis a fantastic opportunity within our ADHD Single Point of Access servicewithin a close-knit and person-centered team. Following the success of theSingle Point of Access pilot, the service has now been commissioned, commencingon 1st April 2026 for a one-year fixed-term contract. The service acts as afront door for all patients in Leeds primarily seeking an ADHD assessment; however,it also supports patients with Autism Spectrum Disorder (ASD) if this isidentified during the screening process.

ThePathway Navigator will receive daily support from the Single Point of AccessPathway Lead and will report to the Service Delivery and Relationship Manager.They will work closely with the Clinical Lead for the Single Point of Access toensure effective day-to-day performance and excellent patient care.


Main duties of the job

Pathwaynavigation includes exploring ADHD symptoms, supporting informed patient choiceabout formal assessment, and ensuring compliance with agreed processes,standard operating procedures, and the Single Point of Access clinicaltemplate.

Job responsibilities

MAIN DUTIES

The SinglePoint of Access Pathway Navigator is responsible for the following;

Complete comprehensive and holisticassessments of need for patients with suspected ADHD who fit the criteria forthe service within agreed timeframes.

Identify patient safety risks and escalte concerns when necessary to the appropriate person, following correct protocolsand within a timely manner.

Develop collaborative wellbeing action planswith service users with ADHD symptoms which promote choice and accessibility.

Discuss all available providers of ADHDassessments enabling clinically appropriate and informed patient choice. In cases where it is agreed to refer, allnecessary patient administration will be completed as per agreed protocols.

Identify any barriers to peopleaccessing support for their ADHD symptoms and work with individuals to overcomethese.

Use a range of techniques and interventions to support people with ADHDsymptoms, to better understand themselves from a strengths and challengesperspective, to encourage a more positive mindset and ability forself-advocacy.

Monitor and record outcomes within agreed clinical templates, assessmenttools and outcome measures.

Reflect on personal and team practice, participating openly andconstructively in team meetings and huddles, peer supervision, and MDT meetings

Ensure that patient records and other information systems are completedaccurately and within agreed timescales.

Responsible for reporting incidentsaccurately and completing all required training and learning within deadlines.

Ensure timely and comprehensivepreparation for MDT meetings, with all required patient information collated,updated, and shared in advance (as per the MDT standard operating procedure).

Maintain high standards of patientcare, evidenced through positive patient feedback and the timely, effectivemanagement of patients across service pathways.

Achieve agreed service activity,quality, and outcome targets in line with contractual KPIs and organisationalstandards.

Worked within agreed KPIs, escalatingissues promptly when targets cannot be met.

Ensure that support undertaken is meaningful and respectful, promotingself-understanding and self-compassion, and is inline with the individual action plan.

Adhere to organisational policies and procedures relating torisk and patient safety.

Work with team membersand colleagues to continually improve the quality of services within theoverall organisational governance frameworks and service objectives.

Have good written and verbalcommunication skills and adapt their approach for people with ADHD as required.

Have awareness and experience ofworking with and respecting confidential material.


Person Specification


Systems and Compliance

* Proficient in the use of Microsoft Office packages including Outlook, Word, Excel, and PowerPoint.
* Knowledge of working with clinical systems such as SystmOne, AccuRx etc.
* Knowledge of the importance of information governance.
* Understands concepts of risk and incident management.
* Experience of managing highly confidential information in a professional and restricted manner.
* Working knowledge of administration processes and systems
* Proven ability to effectively use recommended document management systems.
* Proven ability to maintain and implement methods and techniques for the organisation, storage and version control of information in accordance with regulations, policy and auditing requirements.
* Willingness to learn and develop advanced skills in relevant working area.

Experience

* Lived experience and/or experience of working in a service that directly supports neurodivergent individuals.
* Experience of directly working with members of the public in a busy environment.
* Experience of directly working in a service that supports improved health and wellbeing.
* Experience following Standard Operating Procedures, escalation processes, and service KPIs.
* Capable of managing own workloads and meeting tight deadlines agreed with line manager in line with contractual expectations.
* Ability to work effectively in a challenging environment whilst meeting deadlines and maintaining high quality standards.
* Proven ability and experience of prioritising and organising a large and busy workload and demonstrable experience of planning activities for self or as part of a team.
* Demonstrable experience of working to reduce health inequalities and supporting underserved groups to access services and achieve positive health-based outcomes.


Qualifications

* Degree or foundation degree in a relevant field
* (Psychology, Health & Social Care, Mental Health, Occupational Therapy, Social Work, Education, etc.)
* Coaching, counselling, or motivational interviewing training
* Health coaching qualification
* Evidence of continued professional development, relevant to neurodevelopmental or mental health services
