Torus is seeking a Housing Coordinator to join our team in St Helens, working within the multi-disciplinary Front Door Contact Cares Team on a 12-month contract. In this role, you’ll be the key housing specialist, providing advice, signposting, and practical solutions to support patients and residents. You’ll play a vital part in reducing hospital delays, preventing homelessness, and helping people return home safely and sustainably.
What you'll be doing:
* Acting as the single point of contact for all housing-related issues within Contact Cares.
* Working with hospitals, support centres, and local partners to reduce re-admissions and delayed discharges.
* Supporting patients and families by finding practical housing solutions, including re-housing, repairs, adaptations, and access to assistive technology.
* Providing clear advice and signposting on housing services, welfare benefits, and support available across Torus, local authorities, and the wider voluntary sector.
* Collaborating closely with colleagues across health, housing, and social care to develop new initiatives and improve outcomes for service users.
* Recording outcomes to demonstrate the impact and value of the Housing Coordinator role.
What we are looking for:
* Evidence of working in the housing sector
* Proven experience managing a caseload and collaborating effectively with multiple agencies.
* Skilled in crisis management, risk mitigation, and making sound decisions in challenging situations.
* Empathy, excellent communication, and the ability to work with patients, families, and professionals in sensitive situations.
* A proactive and person-centred approach to finding solutions.
* The ability to work independently and as part of a multidisciplinary team.
Interview Process
* Candidates will undergo in-person interviews week commencing 22nd September, and will consist of a competency-based interview
