Job Title: Clinical Pharmacist
About the Role:
We are seeking an experienced Clinical Pharmacist to join our team. The successful candidate will be required to work in A&E or Women & Childrens' Services, providing clinical pharmacy services to inpatients and outpatient services.
Key Responsibilities:
* Dispensing: Ensuring accurate and safe dispensing of medications to patients.
* Prescribing: Collaborating with prescribers to optimize medication selection and dosage.
* Manufacturing: Preparing and quality-checking sterile medications.
* Purchasing and Testing: Acquiring and ensuring the quality of medicines.
* Stock Management: Managing hospital pharmacy stock levels and ensuring adequate supplies.
* Quality Control: Implementing quality control measures for manufactured and purchased medications.
Patient Care:
* Counselling: Providing patients with information about their medications, including dosages, potential side effects, and how to use them correctly.
* Monitoring: Monitoring patient progress and effectiveness of medications.
* Medication Reconciliation: Ensuring accurate and complete information about patient medications during transitions of care.
* Adverse Drug Reaction Reporting: Identifying and reporting medication errors or near misses.
* Patient Education: Educating patients about their drug therapy and promoting adherence.
Collaboration and Support:
* Clinical Input: Providing clinical pharmacy services to inpatients and outpatient services.
* Teamwork: Working closely with other healthcare professionals, such as doctors and nurses, to optimize patient care.
* Research and Development: Participating in research and development of new medications and therapies.
* Drug Budget Management: Contributing to the management of drug budgets and resources.
* Guideline Development: Developing and implementing guidelines for medication use.
* Training: Providing education and training for junior pharmacists and other healthcare professionals.
