Specialist District Nurse – Out of Hospital Care Collaborative
South Warwickshire University NHS Foundation Trust is recruiting four Specialist District Nurses to lead and shape community nursing across four distinct localities. These roles offer the opportunity to develop, implement and evaluate care programmes, working autonomously and collaboratively within a Place‑Based Team.
Key Responsibilities
* Assess and manage complex health needs within your locality.
* Lead the development, implementation and evaluation of care programmes.
* Work autonomously and collaboratively with a Place‑Based Team to deliver high‑quality, person‑centred care.
* Provide clinical leadership and supervision for all patients within the community nursing caseload.
* Ensure the highest standard of evidence‑based clinical care across the nursing team.
* Support peers, deliver performance development reviews, clinical supervision and recruitment across the team.
* Collaborate with multidisciplinary teams to establish a whole‑system approach to patient management in the area.
* Focus on achieving and demonstrating overall improvements in health care delivery.
Qualifications & Experience
* Registered Nurse with substantial community experience managing patients with complex needs.
* Advanced clinical assessment skills and prescribing authority.
* Completion of the DNSPQ qualification and status as an independent prescriber.

Kenilworth & Warwick
