Job summary
HertsOne GP Federation has an exciting opportunity has become available for a specialist nurse to join a forward-thinking, passionate leg ulcer service covering Southwest Hertfordshire localities, on a full or part time basis, for a minimum of hours per week. This is an ideal role if you are looking to improve your work/life balance as clinics run Monday to Friday, day shifts only, with flexible hours available. This role is offered initially on a 12 month fixed-term contract.

You will be working in a small supportive dynamic nurse led team responsible for assessing and delivering care for patients in a clinic setting. You will work closely with other multidisciplinary professionals where effective communication is essential.
Main duties of the job
1. Being responsible for your own caseload and clinics within South and West Herts, delivering best practice standard of care and encouraging self-care.
2. Promoting a leg ulcer service philosophy which embraces a holistic approach to patients, relatives, and carers.
3. Acting as a resource to practice nurses and community nurses through education to ensure continuation of appropriate leg ulcer care for patients in other environments home, hospital.

Qualifications
Essential
4. A relevant nursing degree.
5. Current NMC registration with an active PIN.
6. Professional Development in the Management of Leg Ulcers qualification (formerly ENB N18)
7. Mentorship and Preceptorship qualification (formerly ENB 998) or equivalent Practice Teaching (CPAD) qualification (or willingness to undertake).
Experience
Essential
8. 3-4 years post qualification experience or equivalent.
9. Experience in primary care.
10. Evidence and understanding of leg ulcer management and awareness of current research.
11. Car driver and current driving licence essential or access to a car (unless you have a disability as defined by the Disability Discrimination Act 1995).
12. Evidence of continuing study in leg ulcer management/ tissue viability.
13. Good verbal and written communication skills.
14. Supervisory skills.
15. Advanced assessment skills
Desirable
16. Nurse prescriber is desirable (or willingness to undertake).
