Children in care face a range of mental and physical health challenges and they require professionals who have understanding of the unique and diverse challenges that they face. They have some of the poorest health outcomes of all children in the country. This role would be primarily to undertake initial health assessments for children in care.
We are looking for staff to support this service and to join our innovative Cambridgeshire Community Paediatric Team. We are a very supportive and committed team, actively involved in service improvement, teaching and research. Staff are well supported and there are opportunities to develop expertise and contribute to wider aspects of our community paediatric service across Cambridgeshire depending on expertise.

Main Duties and Responsibilities
Undertake statutory looked after children health assessments including those with complex healthcare needs.
Formulate a comprehensive, individual healthcare plan.
Identify and lead on relevant audits of service provision, including multiagency audits in conjunction with others.
Support our wider community paediatric work across Cambridgeshire, depending on service needs and applicants expertise.
Communication
Good communication skills to allow effective work within a team of clinical and non-clinical staff in both health, education and social care professionals in order to fulfil the role as described.

Maintaining own professional development and requirement to take part in appraisal and KSF process.
Clinical and Practice Governance
Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.
Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.
The post holder must adhere to the Trust risk assessment and risk management processes.
