Community Support Worker

The closing date is 21 November 2025

Location: Middlesbrough Treatment and Intervention Team – Parkside Mental Health Resource Centre, Middlesbrough. Working hours 09:00-17:00 Monday‑Friday.

This role involves supporting the multidisciplinary team in delivering community‑based mental health care, improving the lives of people with complex mental health disorders, and promoting recovery and wellbeing.


Main duties

* Compassionately meet the needs of patients and their carers.
* Maintain safety, privacy and dignity of all patients, respecting cultural and spiritual beliefs.
* Recognise signs of patient harm, abuse or neglect and follow safeguarding policies.
* Undertake delegated activities within own competency, including risk assessment and intervention plan implementation.
* Complete patient assessment tools and document accurately in care records.
* Identify changes in patients' presentation and report appropriately.
* Accompany and support patients in various community settings, as delegated and according to risk assessment.
* Provide health education and advice on health promotion to patients and carers.
* Act as an advocate for patients/carers and promote a positive image of people with mental ill‑health.


Qualifications and experience

* Diploma in Health and Social Care Level 2 or equivalent.
* Achievement of Trust’s Healthcare Assistant competencies within the first 6 months.
* Previous experience as a Band 2/3 Healthcare Assistant or equivalent.
* Experience working with people with mental/social health needs or learning disabilities.


Skills and knowledge

* Effective verbal and written communication.
* Competency in record keeping and time management.
* Ability to prioritise tasks.
* Competence in venepuncture and ECGs.
* Person‑centred care and awareness of legal frameworks, including Mental Health legislation.
* Knowledge of Microsoft Office applications.


Personal skill set

* Good verbal and written communication skills.
* Competency in record keeping.
* Time management and prioritisation.


Additional information

Applicants must be able to travel independently and have access to a suitable vehicle. Adjustments can be considered in accordance with the Equality Act 2010.

Required clearance: DBS check under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
