Job Title: Adult Social Worker - CHC Tea m Location: Blackpool (Hybrid) Pay Rate: £30.66 per hour Job Description: Liquid Personnel is recruiting an Adult Social Worker to join its client’s Continuing Health Care (CHC) team, based in Blackpool. What will your responsibilities be? In this role, you will: Monitor and review CHC provision, including joint reviews and participation in Decision Support Tool meetings. Undertake statutory social work duties, including Care Act 2014 assessments, safeguarding interventions, and Court of Protection work. Work collaboratively within a multidisciplinary CHC team to support health colleagues and deliver high‑quality outcomes for adults in the Blackpool community.
