Job Overview
The post holder will be based within the Dementia Outreach Team at Airth GP Practice. The role is 37 hours per week.
The Dementia Outreach Team's aim is to provide intensive intervention to people with a confirmed diagnosis of dementia and their carers, particularly during, or at risk of entering, a crisis situation. The post holder will work collaboratively with health and social care professionals across primary, secondary and third‑sector community settings to meet the mental health needs of people living with dementia and promote patient‑centred care across the Forth Valley area.
Key Responsibilities
* Work across primary, secondary and third‑sector community settings to support people with dementia and their carers.
* Deliver intensive interventions in crisis situations and when at risk of entering a crisis.
* Collaborate with health and social care professionals to promote patient‑centred care.
* Travel throughout the service area as required.
* Maintain membership of the Protecting Vulnerable Groups (PVG) Scheme.
Qualifications and Experience
* Registered mental health nurse educated to degree level.
* Experience in an older adult mental health setting.
* Sound knowledge of dementia and associated conditions.
* Excellent communication skills, time‑management and ability to work autonomously.
* Post‑graduate qualifications relevant to the area, or willingness to work towards them, are desirable.
