We invite you to apply for the Band Mental Health Practitioner position in the South Memory Assessment Service, Cambridge and Ely. We are looking for someone with a keen interest in dementia and older people’s care. This post is open to registered nurses with the right attributes to enhance the developing Memory Assessment Service.
You will be contributing to the assessment of memory functioning and participating in the treatment process including post-diagnostic support. You will support service users and carers, offering information and signposting them to other agencies. You will work closely with colleagues from the multi-disciplinary team, psychiatrists, nurses, psychologists and others. We will provide the necessary support to ensure your development within your role

Main duties of the job
To work within the integrated multi-professional team and the Memory Assessment Service.
To be accountable for contributing to assessing, interpreting, planning and implementing treatment and care to patients within professional guidelines, with the support of the Band mental health practitioner.
To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making.
Provide support and education to peers, new staff, non-registered staff and Students.
Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.
