Overview
The successful candidate will be assessing and developing individualised, appropriate and timely discharge plans for patients. They will collaborate with medical, nursing and therapy teams to assess discharge needs early in the admissions process, liaising with community services, social care and families to ensure continuity of care, providing clear communication between hospital and external agencies. They will monitor and reduce delayed discharges and support patient flow across the Unit. A pro-active approach to discharge planning and good communication skills are essential, with the ability to work flexibly to meet the needs of the service. They should be able to problem solve complex situations and recommend appropriate action, often under pressure.
Responsibilities
* Assess and develop individualised discharge plans that are timely and appropriate for patients.

* Communicate effectively and work flexibly to meet service needs.
* Problem solve complex situations and recommend appropriate actions, often under pressure.
Qualifications / Requirements
* Competent IT skills and the ability to use a range of communication styles including face-to-face, telephone, email and MDT working.
* Ability to prioritise workload and work to deadlines using initiative.
* Some experience of working within a healthcare setting and a good understanding of community services.
* A willingness and commitment to learn, with a passion for delivering high-quality patient care.
