ASC Social Worker (hours)

Responsibilities
 * Conduct thorough assessments of clients' needs and circumstances.
 * Develop, implement, and review individualized care plans in collaboration with clients and their families.
 * Provide emotional and practical support to clients to help them navigate their challenges.
 * Advocate for clients' rights and needs within the social care system.
 * Collaborate with healthcare professionals, community organizations, and other stakeholders to facilitate comprehensive care.
 * Maintain accurate and confidential client records in compliance with legal and ethical standards.
 * Stay informed about social care policies, practices, and resources to effectively assist clients.
Requirements
 * A recognized qualification in social work (e.g., BA/MA in Social Work).
 * Valid social work registration and a clear understanding of relevant legislation.
 * Experience in adult social care or a related field is preferred.
 * Strong communication and interpersonal skills to effectively engage with clients and their families.
 * An understanding of mental health issues, disabilities, and social challenges faced by clients.
 * Proficiency in case management and the ability to maintain detailed records.
 * Demonstrated ability to work collaboratively within a multidisciplinary team.
